Port Charlotte, FL – $85,500.00

• Assess and treat mental health, substance use, or co-occurring disorders in children and families in the community on the Family Support Team serving Charlotte County.
• Complete thorough comprehensive assessments for adults and children, as scheduled and/or needed.
• Develop and implement recovery-focused service plans with measurable objectives.
• Provide evidence-based brief and solution-focused individual and family therapy.
• Maintain accurate and timely documentation per agency standards.
• Provide individualized services in the community, including in homes and other community settings.
• Carry a caseload and provide case management services to patients as needed as part of a multidisciplinary team.
• Attend and participate in Team Clinical Staffing and treatment planning meetings twice weekly.
