Central Florida Inpatient Medicine seeks a Home Visit APRN or PA to serve as day-to-day clinical leader, guiding patient transitions from acute care to home and directing a multidisciplinary team.
You will perform 6-8 preventive home visits daily to optimize chronic conditions, assess home environments, educate patients and caregivers, and develop proactive care plans. Coordination with PCPs, hospitalists, SNFs, and community services helps smooth transitions and prevent readmissions.
