Base pay range$30.00/hour – $34.00/hour
Schedules and Incentives- 6:00 pm–6:30 am with rotating weekends (full-time)
- Rate: $30–34 based on years of experience
- Offering shift differentials
- Sign‑on Bonus: $5,000.00
- Patient Assessment and Care Planning:
Assist in assessing the health status of patients and contribute to the development and modification of individualized care plans. - Direct Patient Care:
Provide safe, effective nursing care for multiple patients within the facility’s healthcare program. - Implement patient education initiatives based on established teaching plans.
- Nursing Scope of Practice:
Perform delegated nursing tasks consistent with educational preparation and within the scope of practice. - Use a problem‑solving approach to make informed clinical decisions.
- Care Coordination and Monitoring:
Oversee onsite and off‑site patient scheduling in collaboration with the care team; ensure follow‑up care for patients, including telemedicine appointments and consult processes in accordance with policy and contract requirements. - Quality Improvement and
Collaboration:
Participate in Quality Improvement initiatives and Patient Care Committee meetings; coordinate with the Regional Medical Director, site medical providers, Regional Nursing Director, and other site nursing leaders to ensure seamless patient care. - Documentation and Other Duties:
Monitor CARES and ensure proper coordination as needed; perform additional nursing duties as assigned to support patient outcomes and organizational goals.
- Graduate from an accredited Licensed Practical Nursing program.
- Current licensure as a Practical Nurse (LPN) in the state of employment.
- Active CPR certification.
- Familiarity with state laws and regulations governing LPN practice.
- Successful completion of initial and annual clinical competencies to demonstrate required skills for the role.
- Ability to pass a background check, security clearance, and drug screening.
