$30.00/hour – $34.00/hour

• 6:00 pm–6:30 am with rotating weekends (full-time)
• Rate: $30–34 based on years of experience
• Offering shift differentials
• Sign‑on Bonus: $5,000.00

• Patient Assessment and Care Planning:
  Assist in assessing the health status of patients and contribute to the development and modification of individualized care plans.
• Direct Patient Care:
  Provide safe, effective nursing care for multiple patients within the facility’s healthcare program.
• Implement patient education initiatives based on established teaching plans.
• Nursing Scope of Practice:
  Perform delegated nursing tasks consistent with educational preparation and within the scope of practice.
• Use a problem‑solving approach to make informed clinical decisions.
• Care Coordination and Monitoring:
  Oversee onsite and off‑site patient scheduling in collaboration with the care team; ensure follow‑up care for patients, including telemedicine appointments and consult processes in accordance with policy and contract requirements.
• Quality Improvement and
  
  Collaboration:
  
  Participate in Quality Improvement initiatives and Patient Care Committee meetings; coordinate with the Regional Medical Director, site medical providers, Regional Nursing Director, and other site nursing leaders to ensure seamless patient care.
• Documentation and Other Duties:
  Monitor CARES and ensure proper coordination as needed; perform additional nursing duties as assigned to support patient outcomes and organizational goals.

• Graduate from an accredited Licensed Practical Nursing program.
• Current licensure as a Practical Nurse (LPN) in the state of employment.
• Active CPR certification.
• Familiarity with state laws and regulations governing LPN practice.
• Successful completion of initial and annual clinical competencies to demonstrate required skills for the role.
• Ability to pass a background check, security clearance, and drug screening.
