Position

Summary

Perform routine daily care to Assisted Living and/or Memory Care residents in areas of personal care activities, homemaking, escorts, and other related supportive services essential to achieving a high quality of life for residents living at High Point Center for Care.

Work Schedules: Full Time Evening and Night;
Part Time All Shifts (Morning, Evening, and Night)

Essential Job Functions
  • Assist all residents with a high quality of Activities of Daily Living (ADLs), including bathing, dressing, grooming, shaving, incontinent care, skin/foot care, oral hygiene, and height and weight checks per policy.
  • Actively promote independent life-style of residents.
  • Make beds in the morning and turn down in the evening.
  • Change bed linens weekly or as needed; complete flat linens per shift as assigned.
  • Complete resident’s personal laundry as needed.
  • Ensure residents have supplies and inform the nurse, Healthcare Navigator, or Director of Health Services if supplies are needed.
  • Assist residents during mealtime: supervise meals, escort to meals, cut food, encourage eating, etc.
  • Utilize special program components offered by the community.
  • Assist with certain treatments as ordered by residents’ medical provider.
  • Promote a calm and positive atmosphere.
  • Complete and document shift assignments; report to nursing supervisor if unable to complete.
  • Inform residents of activities, events, and appointments to encourage participation; escort as needed.
  • Document resident status accurately as per policy.
  • Conduct routine checks on residents in their apartments.
  • Receive report from Nurses, Healthcare Navigator, Supervisor, and/or Health Services Director at the beginning of each shift.
  • Attend department meetings as needed.
  • Follow resident Care Plan and communicate changes to nurses, Healthcare Navigator, Supervisor, and/or Health Services Director.
  • Communicate any changes in residents’ mental or physical condition to the nurse on duty, Healthcare Navigator, and/or Health Services Director.
  • Monitor Wander System and E-call Alarms; respond promptly to alarms and emergencies.
  • Provide emergency care within scope of practice.
  • Be familiar with and follow community policies.
  • Support creating a culture where decisions drive top-line revenue and occupancy.
  • Perform essential functions of the job in a professional manner.
  • Other duties as assigned by Health Services Director.
  • Salary: The salary range for this opportunity is $20.00 - $21.00.
