De Zorgcirkel is op zoek naar een zorgcoördinator in Purmerend die het zorgteam ondersteunt en zorgt voor een veilige, betrokken werkomgeving. Je werkt 36 uur per week en coördineert, begeleidt en verbindt tussen bewoners, familie en behandelaren onder begeleiding van de zorgmanager.
Je signaleert verbeterpunten, denkt mee over de ontwikkeling van de locatie en levert een voorbeeldige persoonsgerichte zorg.
