Day Kimball Health is hiring for a full‑time Physical Therapist to join our outpatient clinic in Putnam, CT.
Location: Outpatient Putnam, CT
Schedule: Day Shift (8:00 a.m. – 4:30 p.m.)
Hours: 40 Hours Per Week

As the Physical Therapist (PT) under the general direction of the Physical Medicine Director, you will organize and administer prescribed physical therapy programs to reduce or prevent disability and help patients achieve maximum functional potential.
Job Responsibilities- Evaluate and treat patients as prescribed by referring physicians.
- Record treatment, response, progress, and discharge information in patient charts or electronic systems.
- Educate patients, families, and significant others about conditions, treatment, and home programs.
- Consult routinely with physicians and other practitioners in person, by phone, or in writing regarding progress or changes in treatment plans.
- Orient, instruct, and direct assistants, aides, and students.
- Maintain professional growth through attendance of department meetings, in‑service trainings, and educational conferences.
- Assist with planning and implementing new programs.
- BS, MS, or DPT degree from an accredited program in Physical Therapy.
- Licensure by the state of Connecticut.
- Current AHA Healthcare Provider CPR certification or equivalent within 3 months of hire.
- Minimum of one‑year clinical experience preferred.
- Assess referral documentation and medical records to determine appropriate physical therapy treatment.
- Perform tests and measure strength, motor development, sensory perception, functional capacity, respiratory and circulatory efficiency.
- Plan and prepare written treatment programs with realistic, objective, short‑ and long‑term goals.
- Deliver individualized physical therapy programs including therapeutic exercise, moist heat, traction, manual therapy, cold, gait training, hydrotherapy, ultrasound and electrical stimulation.
