Community Health Resources is seeking an Intake Therapist to join our New Life Center and Milestone at Putnam, CT. This full‑time role supports women with substance abuse and trauma through assessment, individualized treatment planning, and crisis intervention across varied care settings.

Monday – Friday: 8:30 am – 5 pm;
Tuesday: 11 am – 7:30 pm

Programs

Milestone is a 7–14‑day stabilization intensive residential treatment center for women with substance abuse, trauma and mental health issues, offering individual and group counseling, case management and aftercare planning.

New Life Center is a residential substance abuse treatment program for women with infants, toddlers under 4 years or expectant mothers, facilitating healthy parental bonding while providing medical care for mothers and infants.

Responsibilities
  • Provide assessment and treatment interventions consistent with the program model.
  • Conduct clinical and risk assessments.
  • Collaborate with individuals to create individualized, person‑centered treatment plans.
  • Demonstrate competency in age‑related or specialty issues and developmental needs for each population served.
  • Work with clients to develop appropriate discharge plans utilizing natural support.
  • Maintain collaborative relationships with family members and other service providers.
  • Provide crisis assessment and intervention to monitor status of clients in crisis.
  • Offer care in office, home, residential, and/or community‑based settings based on program model and setting.
  • Maintain client and staff records according to agency policy.
  • Cover for team members during periods of absence.
  • Participate in weekly supervision, consultations, and meetings.
  • Transport clients using agency vehicles and/or personal vehicles based on program model and setting.
  • Participate in all required CHR training and obtain and/or maintain certification in an EBP if applicable.
  • Prioritize safety and physical presence in residential programs, serving as a full member of the milieu.
  • Perform additional responsibilities as required.
Qualifications

Education: Master’s degree or higher in a related field.

Experience: Experience working with trauma, mental illness, serious emotional disorders and/or substance use disorders preferred.

Licensure/Certification/Registration: Required credentials: LMSW, LPC‑A, LMFT‑A. Preferred: LCSW, LMFT, LPC, Licensed Psychologist. LADC may substitute for LCSW, LMFT, LPC or Licensed Psychologist in some programs. Valid driver’s license required where transportation is needed.

Compensation

Starting at $54,996 per year, commensurate with experience. Rates are determined at the time of offer and may include program/location differentials and sign‑on incentives.
