We believe in developmental and training opportunities for our staff and therefore are currently seeking to recruit highly motivated, passionate Band 5 Community Nurses who are keen to develop and work towards one of the following roles;
- Band 6 District Nurse by undertaking the District Nurse Qualification.
- Chronic Condition Case Manager by undertaking disease specific training to provide evidence based care to those individuals with highly complex needs.
- Nurse Practitioners by undertaking a modular approach to attain the clinical skills required.

The West Division of Betsi Cadwaladr University Health Board delivers District Nursing Services across the Counties of Anglesey and Gwynedd working in an integrated way with partner organisations to deliver high quality evidence based care in order to maintain patients in their own homes and communities, averting hospital admissions, empowering the patient towards self management and implementing seamless patient centred care and treatment.
You must be able to travel between patients homes both within towns and rural areas in a timely manner. The ability to speak Welsh is desirable for this post;
