Coordonnateur de l’admission / Care Coordinator

Working Conditions:

Hourly rate: $20–$24.10

Key Responsibilities / Accountabilities and Time Allocation
  • Effective Referral Coordination and Data Entry – 60%
  • Reviews funder case documentation for completeness, identifies missing information, requests missing data, monitors and follows up on medical information prior to assessment.
  • Screens for conflicts of interest.
  • Accurately schedules assessments and treatments, confirms clients in the scheduling system, and ensures timely service in accordance with SLAs.
  • Uploads referral and medical information, completes client profiles in Clinic Master, and follows up with referral sources for any missing data.
  • Behaves with integrity and adheres to best practice and CBI policy, including privacy policy.
  • Collaborates with the Team Lead to resolve execution gaps and identify improvement opportunities for the team, program, and service.
  • Provide exemplary customer service – 30%.
  • Communicates with clients and funders promptly regarding scheduling and information management.
  • Employs effective customer‑service strategies for phone, email, and message handling, and follows up in a timely manner.
  • Maintains knowledge of privacy, confidentiality, and code of conduct policies.
  • Builds relationships with referral sources to become a trusted advisor, leveraging comprehensive knowledge of their needs.
  • Facilitates navigation of service offerings with referral sources and clinical teams to ensure appropriate booking.
  • General Administration – 10%.
  • Works to meet or exceed funder referral requirements regarding process management and operational efficiency.
  • Communicates to the Team Lead any barriers in achieving KPIs.
Knowledge,

Education and Experience
  • 2+ years of customer‑service experience.
  • Post‑secondary education in medical office or business administration, or equivalent.
  • Successful experience in multi‑site client booking.
  • Healthcare experience is an asset.
  • Strong professional communication skills in English (French is an asset).
  • Customer‑experience, planning, organizing and multitasking skills.
  • Analytical ability and ability to solve complex problems and make decisions.
  • Ability and flexibility to work with multiple stakeholders.
  • Ability to work in a team and independently.
