In the dual role of clinic nurse and Survivorship Coordinator, you will work closely with physicians and the care team to ensure appropriate care for Radiation Oncology patients. Your responsibilities also include evaluating patient outcomes and creating survivorship plans for the Cancer Program. Active participation in weekly Tumor Board meetings is essential to this role, promoting collaborative care.
Key Responsibilities:
• Oversee patient care and evaluate outcomes effectively
• Create and deliver Survivorship Care plans
• Coordinate and track the Cancer Program
• Support the physician and healthcare team as a resource
• Facilitate weekly General Tumor Board meetings
Requirements:
• Preferably experience in Radiation or Medical Oncology
• Interest in Survivorship care is required
• BCLS certification is essential
• Oncology Nurse Certification strongly preferred
• Valid RN license in Illinois
