Enhance community health with the Wemindji Community Miyupimaatisiiun Center as a Coordinator of Primary Care Services. Drive multidisciplinary collaboration for effective primary care and current services delivery.
This vital role reports to the Director CMC and focuses on the strategic planning, organization, and support of integrated primary healthcare. Candidates must demonstrate strong skills in chronic disease management, preventative health strategies, and psychosocial support while ensuring cultural safety and accessibility for clients. You will oversee the coordination of vital services aimed at improving health outcomes.
Key Responsibilities:
• Manage integrated chronic disease prevention programs
• Oversee primary health care service delivery
• Coordinate triage and emergency services effectively
• Facilitate communication among CMC service providers
• Monitor adherence to healthcare regulations and standards
Requirements:
• Bachelor’s degree in Nursing and O.I.I.Q membership
• Five years of experience in primary care program management
• Knowledge of MSSS regulations and community health services
• Strong interpersonal and leadership skills
• Fluent in English with Cree or French as an asset
