This is a physician‑led group surrounded by excellent support, financial stability, and a strong focus on patient and provider wellness. You will play a pivotal role in transforming healthcare delivery through a collaborative team approach with our population team health. Our clinic is currently focused on primary care services with chronic disease management, Medicare wellness, physicals, well‑child examinations, acute care visits and minor in‑office procedures.

• Outpatient setting focused on clinical excellence and patient satisfaction within a value‑based model
• Emphasize quality outcomes through collaborative care with our population health team
• Monday through Friday, with occasional Saturday
• Light call: phone call consult only
• 36 hours of patient contact; 4 hours admin time
• Support includes Medical Assistants, integrated team of specialists, Pharm D, onsite lab, and additional resources

• Are Board Certified or Board Eligible in Family Medicine or Internal Medicine
• Prefer to see adult patient population
• Have experience with or strong interest in value‑based care, quality improvement, and population health
• Are patient‑focused, friendly, and willing to collaborate in a team environment
