Physicians - Moonlighting

Position: Physicians - Moonlighting -

* * Physicians transitioning from residency (3rd year) to practice are encouraged to apply.
* * Responsibilities
* * Function autonomously to care for patients with acute, critical and chronic health conditions.

* Coordinate care management proactively working with the healthcare team and facility partners to produce good outcomes.

* Provide medical oversight of patient care and rehabilitation.

* Ensure proper progress on plan of care and identify any early warning signs for readmissions during the critical, immediate post-discharge period

* Coordinate appointment for evaluation by specialist as needed for patients during their stay at the care facility

* Work individually and in interdisciplinary teams of resources and consultants

* Facilitates patient transition within and between health care organizations to ensure accurate communication among the health care providers

* Record and maintain patient records in timely manner to ensure all services are accurately accounted for and available for review by the interdisciplinary teams

* Timely documentation, discharge summaries, medicine reconciliation, etc. in EMR

* Talk to patients about effectively managing their health, as well as assisting in designing appropriate treatment plans and recommending further care

* Demonstrated problem-solving and workflow management

* High level of clinical proficiency and excellent decision-making skills.

* Ability to work independently and as an effective member of a healthcare team.

* Excellent oral and written communication skills.

* Ability to effectively collaborate and build relationships with patients, families and all care providers

* Proven organizational skills and ability to prioritize effectively.

* Strong computer skills with knowledge and experience with Electronic Medical Records

* Qualifications
* * Current unrestricted licence MD/DO

* Current and unrestricted DEA certificate

* Board certified or board eligible

* BCLS/ACLS desired

Job Type: Part-time

Pay: $200.00 - $300.00 per hour
