Registered Nurse Case Manager Opportunity

A facility near Quincy, Massachusetts is seeking a full-time Registered Nurse Case Manager to join their team. This opportunity comes with a $5,000 sign on bonus as well!

Pay Range: $29.00 – $47.00 / hour

Sign-On Bonus: $5,000

• Collaborate with a multidisciplinary team to create and implement personalized treatment and discharge plans
• Coordinate services that align with patient needs, payer guidelines, and clinical goals to ensure seamless care transitions
• Monitor and improve the patient experience by focusing on quality, timeliness, and continuity of care

• Must be a licensed Registered Nurse (RN) or Licensed Social Worker (LSW / LCSW)
• Must have previous experience as a Registered Nurse Case Manager or Case Manager
