RN Clinical Care Manager - Up to $55/hr

We are seeking a dedicated RN Clinical Care Manager to support individuals with complex healthcare needs through personalized, coordinated care. In this role, you will serve as a clinical guide, ensuring members receive the resources, education, and advocacy necessary to achieve their health goals. This is an opportunity to make a meaningful impact every day while working alongside a dynamic, compassionate team.

This is a fully remote role; however, only candidates living in Massachusetts will be considered due to company policy.

This is a contract position with a pay range between $47-55/hour based on experience.

Key Responsibilities
  • Conduct comprehensive clinical assessments to evaluate patient needs and create care plans
  • Coordinate services across multidisciplinary teams to ensure continuity of care
  • Educate members and families on care plans, treatment adherence, and health self-management
  • Monitor progress and adjust care strategies to meet evolving health needs
  • Ensure compliance with case management principles, regulatory standards, and NCQA requirements
  • Document all interactions and care plans accurately in accordance with organizational and regulatory standards
Requirements
  • Minimum 2 years of professional RN experience in inpatient, outpatient, or differing levels of care
  • Active RN licensure in Massachusetts
  • Proficiency with electronic documentation using healthcare database systems
  • Comfortable with phone-based work while having strong verbal communication skills
  • Must be currently living in Massachusetts
Nice‑to‑Have Experience
  • Previous case management or care management experience
  • Experience working with diverse populations and individuals with complex care needs
  • Familiarity with social determinants of health and care disparity interventions
Preferred Skills
  • Case Management certification (CCM)
  • Certified Diabetes Educator (CDE) or Wellcoach certification
