Community Care is hiring a Case Manager/Care Manager to join our Family Care Program team. This rewarding opportunity is perfect for someone who enjoys team collaboration, community case management, and being part of a mission driven organization. Community Care’s mission is to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community.
This full-time opportunity offers a weekday schedule and a $5,000 SIGN-ON BONUS. Position based in Racine county.
Job Responsibilities
- Manages an assigned caseload of program members in collaboration with an RN Care Manager
- Completes comprehensive member assessments, member centered care plans, & functional screens
- Provides ongoing care coordination and quality assurance monitoring of services
- Maintains required documentation in adherence to established standards.
Job Requirements
- Education:
Bachelor’s Degree in a Human Services or closely related field required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree. - Experience:
One year of experience with older adults or persons with physical and/or intellectual disabilities in a social service capacity preferred. - Certification:
Eligibility for long-term care functional screener certification is required. - Travel:
Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
- Monday to Friday, 8:00 AM–4:30 PM
