FOX Rehabilitation is recruiting a licensed Occupational Therapist to provide one-on-one care in patients' homes or senior living settings in Cottage Grove, WI. You will conduct evaluations, create individualized plans of care, and deliver evidence-based therapy focused on mobility, independence, and quality of life.
You will document patient progress in FOX’s EMR, collaborate with a multidisciplinary team, and educate patients, families, and staff to promote safe aging in place.
