Lotus Case Manager / Intake Coordinator

• Perform case management, utilization review, and community correspondence in an inpatient setting.
• Engage with Lotus referral sources and third‑party payers on a daily basis.
• Coordinate discharge planning and after‑care services for patients.
• Serve as liaison between acute and Lotus adolescent treatment programs.
• Assist with patient discharge preparation, unit graduations, and documentation of discharge plans.
• Communicate effectively with community partners, clients, and staff to ensure continuity of care.
• Prioritize multiple responsibilities and adhere to established protocols and instructions.
• Maintain accurate and timely documentation to meet program and regulatory requirements.
• Apply clinical judgment and skills essential to the role.

• Graduate of a four‑year college or university with a degree in a clinical field, or possess an equivalent combination of education and experience.
• Experience in case management, discharge planning, or a related clinical setting.
• Demonstrated ability to communicate effectively with community partners, patients, and interdisciplinary teams.
• Strong organizational skills with aptitude for managing multiple demands and following instructions accurately.
• Excellent clinical judgment and professional skills.
