Lotus Case Manager / Intake Coordinator

Responsibilities- Perform case management, utilization review, and community correspondence in an inpatient setting.
- Engage with Lotus referral sources and third‑party payers on a daily basis.
- Coordinate discharge planning and after‑care services for patients.
- Serve as liaison between acute and Lotus adolescent treatment programs.
- Assist with patient discharge preparation, unit graduations, and documentation of discharge plans.
- Communicate effectively with community partners, clients, and staff to ensure continuity of care.
- Prioritize multiple responsibilities and adhere to established protocols and instructions.
- Maintain accurate and timely documentation to meet program and regulatory requirements.
- Apply clinical judgment and skills essential to the role.
- Graduate of a four‑year college or university with a degree in a clinical field, or possess an equivalent combination of education and experience.
- Experience in case management, discharge planning, or a related clinical setting.
- Demonstrated ability to communicate effectively with community partners, patients, and interdisciplinary teams.
- Strong organizational skills with aptitude for managing multiple demands and following instructions accurately.
- Excellent clinical judgment and professional skills.
