Position Summary: The Home Health and Hospice RN Case Manager assesses, plans, implements, and evaluates patient care to achieve optimum patient outcomes for patients as appropriate to service area. Age-specific admission and ongoing assessments are completed, documented and used to develop a plan of care. Provides nursing care and individualized care for patients. Utilizes delegation and critical thinking to provide quality service through a team-oriented, collaborative approach to patient care.

Interacts with patients, families, visitors, and all members of the health care team to develop, coordinate, and manage the plan of care. Manages and oversees delivery of patient care such as, but not limited to:
Maintains patient, staff and customer confidentiality. Demonstrates knowledge of the principles of life span growth and development and assesses data reflective of the patient's status. Interprets the appropriate information needed to identify each patient’s requirements relative to age-specific needs and provides care according to departmental policies and procedures. Work involves application of advanced knowledge and skills of patient care. Duties are varied and complex.

Accountable to resolve issues as determined by license. Handles routine issues, follows up to conclusion on matters, while referring only the most complex or higher level issues and problems to the Manager, Team Lead, or Supervisor. Must be able to work in a fast-paced environment with the ability to effectively prioritize and demonstrate multi-tasking skills. Impact of work affects CHCS and Carilion Clinic scorecard initiatives.

Strong critical thinking skills are necessary to interpret laws and regulations related to CHCS and Carilion Clinic. May be exposed to infectious diseases, bio-hazardous and hazardous materials, including regulated medical wastes, and combative patients. Work is performed in a patient care environment and is often fast-paced, requiring use of prioritization skills.
Physical requirements include lifting, prolonged standing, bending, pushing, pulling, reaching, walking, etc. May be asked to provide care at other Carilion facilities to meet patient needs. Some positions may be required to take call. May be required to travel in adverse weather. Care provided in a variety of home settings.
