As a valued member of our team, you will:

• Conduct comprehensive assessments, including medical history, diagnosis, treatment, and health education.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers to provide holistic care.
• Deliver services at the member’s residence, whether at home or in skilled nursing facilities.

We’re looking for candidates who possess:

• A Master’s Degree or equivalent experience with NP or PA licensure.
• Current NP or PA licensure in your state, including prescription authority.
• Board certification in relevant specialties.
• Current BLS, ACLS, or CPR certification.
• 1 year of experience as a Nurse Practitioner or Physician Assistant (new grads are encouraged to apply!).
• A valid state driver's license and proof of adequate automobile insurance.
• Strong computer skills and excellent communication abilities.
