Care Connection Rehabilitation Center is seeking a compassionate and skilled Physical Therapist (PT) to join our interdisciplinary rehabilitation team. This PRN opportunity is ideal for a PT who wants to make a meaningful impact while providing high-quality care in a supportive skilled nursing environment.
ResponsibilitiesEvaluate patients and assess physical conditions, mobility, and functional limitations
Develop and implement individualized treatment plans
Provide therapeutic exercises, stretching, and hands-on treatment
Monitor patient progress and modify treatment plans as needed
Document evaluations, treatments, and patient progress accurately
Educate patients and families on therapy goals, home exercises, and recovery
Collaborate with nursing and other members of the interdisciplinary care team
Doctor of Physical Therapy (DPT) from an accredited program
Current New Jersey Physical Therapist license
Minimum 2 years of experience preferred
Skilled nursing or nursing home experience preferred
Strong clinical assessment, communication, and interpersonal skills
Familiarity with Sigma Care and Net Health preferred
