Overview
The Case Manager, Social Worker MSW, is responsible for providing case management services to select patients in the community. Implements the case management process to assess, plan, implement, coordinate, monitor, and evaluate resources and services to promote optimal health and health outcomes of patients experiencing complex social determinant of health-related barriers or needs. Conducts psychosocial assessments and facilitates interventions to address identified need(s).
Coordinates the assistance necessary to meet patient specific needs relative to placement, basic living resources, home health resources, counseling, and post-hospital support in order to optimize outcomes. The Case Manager serves as a patient/family advocate while acting as a communication link between the provider, patient, family, health care team, community resources, and payers to reduce redundancy and fragmentation of care.
Ensures that necessary services are provided at the most appropriate level of care effectively utilizing allocated resources. Promotes optimal health and self-management through advocacy, empowerment, identification of and connection with resources, facilitation of efficient service delivery, and promotion of a positive patient-PCP relationship. - Coordinate placement, basic living resources, home health resources, counseling, and post-hospital support to meet patient-specific needs.
- Serve as a patient/family advocate and as a communication link among the provider, patient, family, health care team, community resources, and payers to reduce fragmentation of care.
- Ensure services are provided at the most appropriate level of care while utilizing allocated resources effectively.
- Promote health self-management through advocacy, empowerment, resource identification and connection, efficient service delivery, and a positive patient–PCP relationship.
