As an Advanced Practice Clinician (APC) you will be part of a transformative program designed to provide personalized care and support to our patients, including those with complex and chronic needs, all in the comfort of their own homes.

• Ensure delivery of high‑quality clinical and home‑based patient care.
• Assess, develop, and coordinate care options tailored to members, typically 65 and older.
• Provide home and virtual visits (45–60 minutes) for initial visits, annual check‑ups, or additional support needed to address medical and social challenges.
• Promote optimal health outcomes and provide cost‑effective solutions that adhere to Alignment Health’s policies, state, and federal regulations.

• One (1) year of prior clinical or home care experience preferred; experience with older adults (geriatric) patients preferred.
• Master’s degree from an accredited NP program or PA program.
• Active North Carolina state Nurse Practitioner or Physician Assistant license.
• Active RN license and furnishing number (must, upon hire) for NP only.
• Active Nurse Practitioner Board Certification/Physician Assistant Certification.
• Valid NPI number, DEA, BLS, North Carolina driver license, and automobile insurance.

• Yearly bonus up to 7.5% of annual base salary.
• Restricted Stock Unit (RSU) grant up to 15% of annual salary.
• Continuing education reimbursement up to $1,500 per calendar year + 3 CME days.
