Clinical Pharmacist - Cardiology/Critical Care Services

We are seeking a highly skilled and dedicated clinical pharmacist to join our healthcare team. As a clinical pharmacist, you will play a crucial role in providing comprehensive pharmaceutical care to patients in inpatient cardiology and critical care units. You will collaborate with healthcare professionals to optimize medication therapy, ensure patient safety, and improve health outcomes.

- Medication Therapy Management:
- Review medical charts and assess patients' medication regimens for appropriateness.
- Develop and implement individualized treatment plans, including dosage adjustments and recommendations for therapy optimization during rounds or other forms of communication as applicable.
- Promote best evidence‑based practice through medical literature.
- Order entry into electronic health record (EHR) by reviewing and interpreting orders to assure accuracy.
- Clinical Consultations:
Collaborate with physicians, nurses, and other healthcare professionals to provide clinical expertise on medication selection, dosing, drug interactions, and adverse effects. - Respond to pharmacokinetic consults by assessing their appropriateness, developing a plan, and placing orders per institutional policies.
- Answer drug information inquiries from providers and nurses.
- Documentation of all clinical consults in EHR per institutional guidelines.
- Medication Safety:
- Conduct medication reconciliation processes, including reviewing medication orders, verifying appropriateness, and identifying discrepancies or potential risks.
- Implement interventions to minimize medication errors and adverse drug events.
- Adverse drug event and medication error reporting per institutional policies.
- Quality Improvement:
- Participate in quality improvement initiatives and medication‑related research projects.
- Participate in development and implementation of institutional clinical guidelines and protocols.
- Monitor clinical outcomes, evaluate therapeutic interventions, and propose recommendations for process improvement.
- Continuous Professional Development:
- Stay updated with the literature relevant to practice area and health care regulations.
- Attend continuous education sessions provided by our institution, or other local, regional, and national institutions.
- Attend conferences, seminars, and relevant educational programs to maintain professional competence (optional).
- Teaching:
- Participate in teaching pharmacy students and interns on rotations.
- Participate in teaching our PGY‑1 and PGY‑2 pharmacy residents.
- Participate in teaching our providers and nurses through various teaching methods as applicable.
- Regulatory Compliance:
- Adhere to legal and ethical standards related to pharmacy practice.
- Maintain patient confidentiality.
- Follow applicable laws and regulations.
- Patient
Education: - Counsel patients on proper medication use, potential side effects, and precautions. Provide clear instructions and address any concerns or questions raised by patients or their caregivers.
- Pharm
D or BS Pharmacy Degree
- Active pharmacist licensure with the NC Board of Pharmacy, or obtainable before initiation of employment. Recent graduates of ACPE‑accredited schools may fill this role while pending initial pharmacist licensure, but must obtain licensure within 6 months of orientation or sooner if required for operations.
- No prior experience required.
- Proven experience as a Clinical Pharmacist or in a similar clinical role is highly desirable.
- In‑depth knowledge of pharmacotherapy, pharmacokinetics, and drug‑drug interactions.
- Strong understanding of clinical guidelines, evidence‑based medicine, and pharmaceutical care models.
