Overview
Trusted member of and/or has a close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.
The position on the Community Case Management team will support patient engagement with execution of the care plan, work as a team member and to the community case manager, provider and community resources. Will support targeted patient outreach events including outreach events and street outreach.

LicensureCertified Community Health Worker (CCHW) Required
EducationHigh School Diploma or Equivalent Required. Associate's Degree Preferred
Experience1 Year Customer Service Required. Certified Community Health Worker (CCHW) Required
