What You’ll Do
  • Care for medically complex patients, primarily children, where they are most comfortable, at home.
  • Provide personalized nursing care for pediatric and/or adult patients with complex needs
  • Coordinate care plans with physicians, therapists, and other team members
  • Monitor patient progress and update care plans as needed
  • Educate and support families with in‑home care techniques
  • Document visits and care notes in our easy‑to‑use EMR (electronic charting system)

  • CPR certification

A Registered Nurse (RN) administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and the plan of care per state Practice Act. In this role, you will report to the Director of Nursing (DON).

Duties / Responsibilities
  • Develop, deliver/implement, revise, and evaluate individualized patient‑centered care plans under the supervision of a physician
  • Provide case management to an assigned group of patients and ancillary staff
  • Understand and submit required data collection sets (OASIS, PAR, etc.) and other documentation as requested by the state and/or federal government and insurance payers within required time frames
  • Make the initial evaluation visit and regularly reevaluate the patient’s nursing needs per state and payor requirements
  • Act as an advocate for patient welfare and coordinate care between patients, their families/caregivers and/or their authorized representative, the agency and other healthcare providers/facilities/outside agencies
  • Furnish services requiring substantial specialized nursing skill according to competency level
  • Demonstrate problem‑solving abilities and positive interpersonal communication skills
  • Identify care preferences of patients and verify that interventions meet the patient’s needs and their goals of treatment
  • Initiate appropriate preventive and rehabilitative nursing procedures
  • Prepare clinical and progress notes for each patient visit and summaries
  • Attend case conferences as assigned
  • Inform physician and other personnel of changes in the patient’s condition and needs
