Wellness Nurse LPN​/LVN

The Wellness Nurse (LPN/LVN) supports resident health and ensures clinical quality in a senior living environment. Key responsibilities include conducting wellness assessments, monitoring health changes, managing medications, and performing MAR/TAR audits. The role involves communicating with families, physicians, and care teams, as well as documenting care activities to maintain regulatory compliance.

Additional duties include monitoring incidents such as falls, supporting infection control initiatives, and contributing to clinical quality improvements. The Wellness Nurse trains and supports care team members to uphold high standards of resident care.

• Active LPN/LVN license
• At least 1 year of experience in senior living or a clinical setting
• Strong assessment, documentation, and medication management skills
• Excellent communication and problem-solving abilities
