Position Details
  • Compensation: Our RNs average $85K/year
  • Employment Status: Full-Time
  • Schedule: Flexible Scheduling Monday-Friday, 8 AM - 5 PM

Your Role as a Hospice RN Case Manager

As an RN Case Manager
, you will play a critical role in providing exceptional home health and hospice care
. Your responsibilities will include:

  • Comprehensive Patient Assessments - Evaluating the physical, social, and psychological needs of patients.
  • Collaborative Care Planning - Engaging patients, families, and interdisciplinary teams to develop holistic care plans.
  • Patient-Centered Approach - Providing compassionate support to enhance comfort and quality of life
    .
  • Resource Coordination - Managing patient care resources and referrals efficiently.
  • Accurate Documentation - Maintaining meticulous patient records to ensure seamless care.
Skills & Qualifications
  • Current RN License in Nebraska
  • Home Health and Hospice experience preferred
  • Ability to assist with patient transfers using full range of movement
  • Strong verbal and written communication skills
  • Excellent time management and organizational abilities
  • Valid Driver's License and Reliable Transportation
  • Proof of Auto Insurance and Registration
  • Current CPR Certification (we can help obtain)
  • Negative TB Test (we can provide testing)
  • HEP B Record
