If so, Stoney River Ramsey is looking for an RN Memory Care Coordinator to join our leadership team and help elevate the quality of our Memory Care program.

What You’ll Do (AKA: Your Memory Care Leadership Superpowers) Lead the Memory Care Program:- Serve as the clinical leader for the Memory Care neighborhood, ensuring high-quality, person-centered care.
- Develop, implement, and evaluate individualized service plans.
- Partner closely with the Director of Nursing, Executive Director, and interdisciplinary team to promote best practices in dementia care.
- Support residents through all stages of dementia with dignity, patience, and compassion.
- Model and train staff on engagement, redirection, meaningful activities, and non-pharmacological approaches.
- Foster an environment that promotes independence, comfort, and emotional well-being.
- Provide ongoing training, mentorship, and coaching to LPNs, CNAs, Caregivers, Med Passers, and universal workers.
- Ensure team members understand and follow dementia-specific best practices and regulatory requirements.
- Help build a positive, supportive, team-oriented culture.
- Build strong, trusting relationships with families through consistent communication, education, and support.
- Lead care conferences and guide families through care transitions.
- Maintain compliance with state regulations related to Memory Care programming and clinical documentation.
- Participate in audits, surveys, and quality improvement initiatives.
- Ensure the neighborhood remains survey-ready at all times.
- Current RN license in good standing (required)
- Experience working with residents with Alzheimer’s or other dementias (strongly preferred)
- Prior experience in Assisted Living, Memory Care, SNF, or similar setting
- Strong understanding of dementia progression and person-centered care approaches
- Compassionate leadership style with excellent communication skills
- Ability to train, coach, and inspire teams
- Highly organized with strong follow-through
- Proficiency in Microsoft Office and electronic medical records (Point Click Care experience preferred)
Physical Requirements:
- Ability to lift up to 30 lbs and assist with resident care as needed
- Comfortable with walking, bending, and standing throughout the shift
- Ability to maintain a flexible schedule, including occasional evenings/weekends based on resident or staff needs
