Invicta Health has an exciting opportunity for a Clinical coder and summariser to join the team on a part time basis, 30 hours per week.

Main duties of the jobAs a Clinical Coder and Summariser, you will support the practice in becoming a paper-light environment by summarising and reading coding relevant information from patients clinical records onto the practice computer system in adherence to the Practice Summarising Protocol. You will extract information from patients notes and enter it onto their digital record, displaying significant medical problems prominently.

- Summarise and code patient records onto their digital records using read code/SNOWMED codes to a high standard and according to set protocols and timescales agreed by the practice.
- Review medical records and extract information to produce an accurate summary of the patients medical history.
- Use DOCMAN to enter QOF data, ensuring correct SNOMED clinical codes are used.
- Check and integrate medical records received electronically.
- Examine records and documentation for missing information, using medical knowledge to correct mistakes found in existing patient records, including degraded codes.
- Identify and highlight any read coding problems to the GP and/or Practice Manager.
- Adhere to the Summarising Protocol and keep updated with any changes.
- Attend clinical meetings where required, communicate safeguarding updates and review safeguarding registers; manage and maintain safeguarding record.
- Communicate with other practices and healthcare professionals regarding patient documents; collaborate with clinical colleagues and other healthcare professionals to ensure information accuracy.
- Ensure that the practice complies with clinical coding guidelines, including alerting other users of new guidelines or changes to coding protocols.
- Process E‑Letters as required on DOCMAN.
- Undertake filing and retrieving of paperwork to ensure up‑to‑date maintenance; analyse patient records, extracting and accurately recording all relevant clinical information.
- Accurately code information, following coding protocol and using industry recognised codes.
- Carrying out administrative tasks relating to patient records, including filing and document management.

Work collaboratively and professionally with colleagues across Invicta Health, building and maintaining relationships; contribute to the development of services, including processes, templates, and principles; take part in any statutory and mandatory training or other training and development programmes as requested.
