** PAY RANGE:** $75,000 - $125,000/year
** JOB SUMMARY
** Grand Care Health Services is seeking a Physical Therapist (PT) to join our team. The PT is responsible for providing physical therapy services to patients according to a written physician’s plan of care.
** DUTIES AND RESPSONSIBILITY
*** Assess and evaluate therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care.
* Evaluate home environment.
* Assess for muscle strength, mobility, gait, ROM—potential for rehab.
* Direct physical therapy treatment.
* Instruct patients and families/caregivers in the use and care of therapeutic appliances.
* Determine priority needs for physical therapy.
* Report to physician patient’s reaction to treatment or changes in condition.
* Initiate physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient.
* Instruct them as to the goals of the physical therapy program for the patient by participating in case conferences.
* Train patient in the use of prosthetic device.
* Identify patient and family/caregiver needs for other home health services.
* Prepare and submit clinical and progress summaries based on the attainment of goals.
* Participate in discharge planning for patient.
* Prepare and submit a clinical progress summary based on the attainment of goals.
* Provide physical therapy consultation to home families/caregivers.
* Provide in-service education programs for nursing organization personnel.
* Participate in peer consultation process.
* Supervise Physical Therapy Assistants according to organization policy and state regulations.
** JOB REQUIREMENTS
*** CA Physical Therapy License
* 2+ Years of physical therapy experience, home health experience preferred
* Demonstrate good verbal and written communication and organization skills
* Possess and maintain current CPR certification
* Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
