Job Title

RN Care Coordinator – Case Management Social Services

Job Objective

Determines the appropriateness of hospital admission and advocates, coordinates and facilitates the interdisciplinary plan of care to expedite medically appropriate, effective, efficient and timely utilization of resources for maximum patient outcomes. Partners with the charge nurse, social worker, physician and other members of the interdisciplinary team to facilitate safe and timely discharge, and intervenes as appropriate to remove barriers to efficient patient throughput and smooth patient transition.

Applies clinical expertise and medical appropriateness criteria to resource utilization, admissions and discharge planning.

Responsibilities
  • Advocate, coordinate, and facilitate the interdisciplinary plan of care to expedite medically appropriate utilization of resources.
  • Partner with charge nurse, social worker, physician, and other interdisciplinary team members to facilitate safe and timely discharge.
  • Remove barriers to efficient patient throughput and smooth patient transition.
  • Apply clinical expertise and medical appropriateness criteria to resource utilization, admissions, and discharge planning.
Qualifications
  • Education
    :
    Bachelor of Science in Nursing (BSN) or Master’s degree in Nursing.
  • Licensure/Certification
    :
    California Registered Nurse (RN) licensure (required). Preference for Certification in Case Management.
  • Experience
    :
    Two (2) years of experience as an RN in an acute care or case management setting (required).

Sign-On Bonus

Sign-on bonus offered for all full-time positions. Part-time positions receive a prorated sign-on bonus. First installment of the bonus pays out with the first paycheck, and remaining installment pays out at one year of service. Employees must be either part-time or full-time when the second bonus is paid out. Valid for external new hires only. Some restrictions apply.
