RN - Case Management

The RN Care Coordinator is responsible for determining the appropriateness of hospital admission and for advocating, coordinating, and facilitating the interdisciplinary plan of care. This role aims to expedite medically appropriate, effective, efficient utilization of resources for maximum patient outcomes.

• Conduct thorough assessments to determine hospital admission appropriateness.
• Coordinate and facilitate interdisciplinary care plans.
• Advocate for patient needs and ensure smooth transitions of care.
• Arrange post-acute services including home health, hospice, skilled nursing, and more.
• Oversee discharge planning and medication pre-authorizations.

• Minimum of 2 years of Case Management experience.
• Strong acute care experience and familiarity with the EPIC charting system.
• BLS Certification.
