Nurse Navigator

Summary

The Behavioral Health Navigator is a professional who has specialized training and expertise or equivalent experience in navigating the health care system specific to patients with behavioral health concerns and resources for those individuals in the community. Services provided include but are not limited to supportive communication and coaching, education, information and referrals to community resources and support groups, and working with the health care team and patient’s physician(s) to develop a service plan.

The navigator works with the clinical team to identify individuals who have complex health needs. The navigator actively manages the patient’s care in coordination with the patient’s physician and other service providers as well as provides resources and supported well as educates and empowers the patient/family to take an active role in managing the day-to-day challenges of the patient’s individual diagnoses to increase health & well-being.

The navigator will define a working length of stay based on admission diagnosis. Assists the care team in discharge planning coordination and intervention.

  Associate’s Degree in Nursing
• Preferred:
  Master’s Degree

  Registered Nurse License - Current Maryland license or eligibility to obtain Maryland license

  Basic Life Support

• Case Manager Certification
