Overview
An exciting opportunity is available within Adult Care, Housing and Public Health, as an experienced Social Worker working in our Enablement Assessment Team.
As a Social Worker in the Enablement Assessment Team, you will be responsible for undertaking Care Act Assessments to support customers to remain in their own homes as independently and safely as possible, facilitate safe and timely discharges from our bed bases such as intermediate care bed and short stay placements within a 24 hour care setting.
You will be screening safeguarding concerns and completing initial safeguarding enquiries under Section 42 of the Care Act. There will be a focus on safety planning and risk management, making safeguarding personal, empowerment and working with partners, including the Police, NHS professionals and Housing colleagues.
You will support customers to maximise their independence and personal outcomes through the provision of comprehensive advice, information, practical support, signposting and . Focusing on strengths, prevention, early intervention, and the promotion of wellbeing throughout your practice. You will ensure that customers receive the right support, advice and information at the time they need it.

You may be completing referrals to access short term support or assistive technology to help people live independently in their own homes. You will work as part of a wider team and work closely with Wellbeing Advisors, Assessment and Review Coordinators, Occupational Therapy colleagues and Team Managers.
