Job OverviewBand 4 Health and Wellbeing Coach. Health and Wellbeing Coaches adopt a whole‑person approach to address existing issues and promote proactive prevention of new and existing illnesses. They use a non‑judgemental approach, strong communication and negotiation skills, and support personal choice and positive risk taking, ensuring patients understand their own accountability.
DetailedJob Description And Main Responsibilities
- Coach and motivate patients through multiple sessions to identify their needs, set goals, and support them to implement a personalised plan.
- Provide personalised support to individuals to ensure that they are active participants in their own healthcare journey, empowering them to take more control and improve health outcomes.
- Provide interventions.
- Build and maintain relationships with local community groups, other health, social care, and voluntary sector providers.
- Connect patients to community‑based services via collaboration with local groups and providers.
- Support patients to establish and attain goals set by themselves based on what is important to them.
- Work across the perioperative pathway to deliver specific health advice, where appropriate.

QualificationsEssential criteria
- Educated to A level / NVQ Level 3 or equivalent knowledge of healthcare settings acquired through training and experience.
Desirable criteria
- Training in motivational interviewing / coaching.
Essential criteria
- Experience dealing with complex patients, including those in a sensitive or heightened emotional state.
- Experience dealing with highly complex/sensitive issues.
- Experience working in health organisations.
- Understanding importance of stakeholder management and engagement, including communication planning.
