Sanctuary Personnel Ltd is seeking a Social Worker to join the Transfer of Care Hub Team in Warrington. This locum, full-time role offers 37 hours per week with a hybrid working pattern (approx. 3 days in the office).
You will facilitate safe hospital discharges, perform holistic assessments, develop person-centred care plans, and coordinate multi-agency support, while maintaining accurate case records using Mosaic.
