Reading Borough Council is seeking a dedicated Social Care Coordinator to join the Review Team within Adult Social Care. The role involves carrying out annual Care Act reviews, conducting assessments and care and support planning to help adults access services.
You will work with service users, families and multidisciplinary teams to develop person-centred plans, promote preventative services and ensure records are accurate. Hybrid working is offered to support work-life balance.
