Reading Borough Council is seeking a Social Care Coordinator to join the Review Team in Adult Social Care. This is a permanent, full-time role based in Reading with hybrid working—3 days remote—to support flexible arrangements and high-quality care.
You will conduct assessments, reviews and care planning under the Care Act 2014 and the Mental Capacity Act 2005, collaborating with service users, families and multidisciplinary teams to develop person-centred plans and ensure records are accurate
