Are you a Physiotherapist looking for a fantastic community opportunity where you'll be supporting people back to or within their own homes? We are looking for a Physiotherapist to join our friendly Pathway 1 team (D2A) which is part of the wider Intermediate Care service for Somerset. You will gain an in depth understanding of community working and supporting people within their own homes.

The role focuses on assessing patients in their usual place of residence to determine ongoing rehabilitation, care, and equipment requirements, promoting independence and preventing unnecessary hospital admission or readmission. The physiotherapist plays a key role in coordinating care across health and social care services, providing clinical expertise, advice, and support to colleagues within the Pathway 1 team. Working autonomously and as part of a multidisciplinary team.

The post holder contributes to service development, quality improvement, and education, ensuring evidence based practice and adherence to local and national discharge and rehabilitation standards. Due to our positive outcomes and progress we have secured further investment to continue to expand and grow our service and we have a number of new posts available.

Location:

Taunton & West Somerset/Sedgemoor. As part of the selection process, a face-to-face interview will be required to include a practical element. If you have any accessibility needs or require adjustments, please let us know, and we will be happy to accommodate.

You will be expected to carry out specific therapy assessments which will include goal setting and treatment planning, assessing risk, promoting and enabling independent functioning. You will be committed to a person-cantered approach to your work, have an ability to prioritise and work flexibly. Working alongside our provider teams and rehabilitation practitioners. Are you a fully qualified Physiotherapist with an active HCPC (or pending due to qualifying) registration?

Do you have a desire to enhance your post‑graduate training and experience? Do you want to get involved in a pioneering service that welcomes development initiatives and integration projects? Keen to take an active role in developing and nurturing others, both personally and professionally? Hoping to support the leadership of an engaged, friendly and supportive team.
