What you will do
- Undertake comprehensive mental health, cognitive, risk, and capacity assessments for older adults referred to the service.
- Manage a complex caseload and act as Care Coordinator, developing and reviewing person-centred care and treatment plans.
- Contribute to memory assessment pathways, dementia diagnosis processes, and post-diagnostic support for individuals and families.
- Deliver evidence-based psychosocial and therapeutic interventions to support recovery, wellbeing, and independence.
- Work collaboratively within multidisciplinary team meetings and contribute to clinical decision-making and case formulation.
Person Specification
Qualifications
- Registered Mental Health Nurse, Occupational Therapist, or Social Worker.
- Professional qualification recognised within the UK healthcare sector.
- Evidence of post-registration study at Degree, Diploma, or equivalent level.
- Practice Assessor, Practice Supervisor, or Mentorship qualification is desirable.
Registration & Compliance
- Current registration with NMC, HCPC, or Social Work England, as appropriate.
Experience
- Significant post-registration experience within community mental health, older adult mental health, or memory services.
- Experience supporting individuals with dementia, cognitive impairment, and complex mental health needs.
- Demonstrable experience undertaking autonomous assessments, care coordination, and crisis/risk management.
- Previous involvement in supervision, service development, or quality improvement initiatives is advantageous.
