Overview
An exciting career opportunity has arisen for a Community Staff Nurse to join the West Berkshire Community Nursing team. Within the community we are seeing more complex and acute patients than ever before, with increasingly specialist treatment and care being provided by highly skilled staff. You will support, assess and treat patients with a variety of health needs, delivering high quality, evidence based practice.
You will have a passion and enthusiasm to care for patients in their own homes.
- Work in conjunction with the Team Leader, the Community Nursing Band 6, Community Matron and all staff within the team to holistically assess the needs of patients and carers, and to partner with patients and their families to plan, implement and evaluate nursing care programmes.
- Conduct first visits and take immediate action, reporting back to the Team Leader, Band6 or Community Matron for further discussion and assessment.
- Establish working relationships with other professionals and voluntary agencies to achieve health aims.
- Undertake practice assessment and supervisor training, support students and new staff to the service.
- Provide care according to best‑practice evidence, supporting the development of evidence based practice.
- Promote self‑care for patients and embed health promotion into care.
- Demonstrate knowledge and skills in assessment, reassessment, medication management, bladder and bowel care, tissue viability, use and administration of treatment with specialist equipment, palliative care, symptom control, managing acute episodes of care, and reducing hospital admissions, in line with the Band5 competency document.
- Provide care to people with long‑term conditions, undertaking reviews in accordance with the individual patient care plan and with support from more experienced colleagues as required.
- Commit to providing holistic care to patients in their own homes, working independently on own initiative and within a team. Core clinical skills include complex wound care, bladder and bowel management, palliative and end‑of‑life care, and medication administration.
- Registered General Nurse (RGN) with NMC registration.
- Well‑developed written and verbal communication skills.
- Ability to assess, plan, implement and evaluate care.
- Capacity to work flexibly across a 7‑day service requirement.
- Continuous professional development; organisational and time‑management skills.
- Ability to identify risks and act to safeguard the safety of self, patients and others using appropriate aids.
- Strong initiative and ability to work independently.
- Organisational, audit and standard‑setting participation skills.
- Computer skills, including Excel and PowerPoint.
- Willingness to travel across multiple sites in Berkshire.
- Professional behaviour and attitude.
- Full UK driving licence and access to own car.
