The role focuses on long‑term condition management with a salary up to £48,000 and a 25–37.5 hour week, contributing to improved access, outcomes and patient experience.
The ideal candidate is a UK‑registered nurse with post‑registration experience, including cervical cytology and baby immunisations, and strong QoF knowledge.
