We are currently seeking a dynamic Social Care Coordinator to join our Advice and Wellbeing Hub – Occupational Therapy Team. We represent a Local Authority dedicated to enhancing the well‑being of our community and residents.
We seek an individual with excellent communication abilities, adept at building and fostering relationships with colleagues, service users, healthcare providers, and community networks. In the role of Social Care Coordinator at Reading Borough Council, you will play a crucial role within a friendly and cooperative team. You will collaborate with other Social Care Coordinators, Occupational Therapists, and professionals from the voluntary and community sector.

The Advice and Wellbeing HubThe Hub aims to actively promote wellbeing and independence, supporting people to retain or regain their skills and confidence through existing and new support networks, Technology Enabled Care (TEC), equipment and short‑term interventions.
About the RoleIn the role of a Social Care Coordinator in the OT Pathway, your responsibilities will involve making home visits to individuals. You will undertake non‑complex functional assessments of people’s needs and develop and monitor plans in collaboration with service users themselves. The objective is to enhance their independence through a strengths‑based methodology and a person‑centred approach.
- Hold a varied caseload and be expected to work independently and to interpret and analyse varied information or situations to produce solutions.
- Evaluate and suggest basic equipment or TEC, support goal planning, offer short‑term care or enablement, and work closely with care providers, Housing Services, healthcare peers, and the voluntary and community sector.
- Support the team’s Duty function by triaging cases that come into the team with support from Occupational Therapists.
- Work within the legal framework provided by the statute, guidance, policies, and procedures.
- Experience working with people
- Experience working within the Health, Adult Social Care or voluntary sector
