Adult Social Worker – Hospital Team Merton £32.69 per hour Full-time We are currently recruiting for an experienced Adult Social Worker to join a Hospital Team in Merton. The role will involve working with adults in a hospital setting, supporting assessment, discharge planning and positive outcomes for individuals returning to the community or moving into appropriate care.

Key responsibilities:

Undertake Care Act assessments and reviews Support hospital discharge planning Assess care and support needs and develop person-centred plans Work collaboratively with health and social care professionals Safeguard adults and respond to identified risks Liaise with families, carers and partner agencies Maintain accurate records and work within statutory timescales Requirements:
Social Work England registration Qualified and experienced Adult Social Worker

Experience with in hospital or discharge services is desirable Strong knowledge of the Care Act and adult safeguarding Ability to manage a busy caseload effectively Why work through Pertemps?
