Reading Borough Council’s Youth Justice Service is seeking a Transitions Officer to support 17+ young people moving from youth justice into adult services. You will work across community and custody settings, coordinating with Probation and partner agencies to ensure safe, effective transitions.
The role requires strong assessment and interagency collaboration skills, with a focus on reducing reoffending and promoting brighter futures for vulnerable young people.
