• Admissions, discharge, and transfers of residents
• Medication requisitions with pharmacy and physicians, including medication administration
• Follow infection control, emergency, and center policies
• Collaborate with the interdisciplinary team to provide optimal outcomes
• Identify and report abnormal changes in residents' condition to appropriate personnel per policy
• Work under direct supervision in accordance with state Nurse Practice Act, facility policies and procedures, and nursing judgment
• Deliver nursing care to patients/residents requiring long‑term or rehabilitative care
• Collect patient/resident data, observe, and report pertinent information related to care
• Implement patient/resident plan of care and evaluate response per state regulations
• Direct and supervise care given by other nursing personnel in selected situations per state regulations
• Maintain documentation requirements and knowledge of equipment setup, maintenance, and use
• Maintain confidentiality and patient/resident rights regarding all information

• Licensed Practical Nurse (LPN) licensed by the state
• CPR certification required
• Two years of nursing experience preferred
