Position: Licensed Practical Nurse (LPN) - Temporary Part Time (PT) - Michener Hill

Licensed Practical Nurse Mission

Reporting to the Director of Care or Director of Health & Wellness, the Licensed Practical Nurse (LPN) is responsible for providing planned, patient‑oriented, goal‑directed nursing care in compliance with provincial standards of care.

Key Responsibilities
  • Work as a member of the inter‑professional team using Extendicare’s Professional Practice Model and person‑centered care.
  • Compile and keep resident clinical care assessments, care/service plans and documentation accurate, up to date, and communicate changes as required.
  • Implement and evaluate care/service plans, making changes as needed.
  • Assist with scheduling resident care conferences and participate where applicable.
  • Collaborate with the Director of Care or Director of Health & Wellness to facilitate resident move‑ins, move‑outs and transfers.
  • Provide direct resident care, administer medication/treatment, and document care in compliance with regulatory mandates and Extendicare Resident Care standards.
  • Process orders, securely store and administer drugs and narcotics in compliance with regulatory obligations, scope of practice, and Extendicare policies.
  • Facilitate resident and family education.
  • Plan, coordinate, manage and evaluate resident care plans in partnership with resident or decision‑maker, physicians, the nursing and interdisciplinary team, and contracted service providers.
  • Coordinate the nursing team’s resident care assignments to ensure efficient and timely delivery of services.
