Coordinated Entry Outreach Facilitator

Join McMan Youth, Family and Community Services Association for a permanent full-time role focused on helping youths aged 15-24 facing homelessness. Your expertise in frontline human services will play a pivotal role in connecting with youths, assessing their needs, and collaborating with community partners. Strong interpersonal skills and a background in human services will be key to success in this impactful position.

Key Responsibilities:

• Develop trusting relationships with homeless youth

• Collaborate with Adult Coordinated Entry services

• Support youth transitions from intake to housing

• Connect youth with appropriate community resources

• Promote the program within local communities

Requirements:

• Minimum diploma in Human Services, degree preferred

• One year of frontline experience in human services

• Familiarity with SPDAT is an asset

• Valid Class 5 driver’s license and reliable vehicle

• Basic First Aid and suicide intervention training
