In this full-time position, you will provide personalized interventions that focus on the holistic needs of our residents. Your responsibilities will include promoting resident-centered care, conducting assessments, and collaborating with various teams to ensure continuity of care. The role demands excellent communication, initiative, and the ability to assist residents in navigating community resources.

Key Responsibilities:

• Conduct individualized psychosocial assessments for residents

• Facilitate mutual support and therapeutic groups

• Support discharge planning and resource navigation

• Maintain accurate documentation of resident care

• Participate in care conferences and interdisciplinary team meetings

Requirements:

• Bachelor’s Degree in Social Work

• Minimum two years related experience, ideally in gerontology

• Good standing with Alberta College of Social Workers

• Excellent verbal and written communication skills

• Ability to work independently with minimal supervision
